It is necessary for us to obtain registration and insurance information. At the time of appointment scheduling our Patient Service Representatives and/or Nurses will obtain the minimal required patient information needed to schedule an appointment.
Once the appointment is scheduled, you will be transferred to one of our Pre-Registration Representatives to complete the full registration process.
All new patients should report to the New Patient Registration Office located just inside the main entrance 30 minutes prior to your first appointment. This time will be used to initiate your medical record, input or review your pertinent demographic and insurance information into the computer, and sign necessary forms.
New & Existing Patients
Please know it is YOUR personal responsibility to notify the clinic of any change to your name, address, phone numbers, change of employer, emergency contact name, or insurance information as soon as the change occurs. Please have your insurance card and any other necessary insurance billing information with you.
Please plan to arrive in the reception area 15 minutes early for your appointment. Please check in at the desk so that we can review your registration information to make certain it is current and accurate and collect any co-payments that are due with your insurance plan.
